Each grant recipient must complete a form W-9 to receive grant funds. Forms MUST be completely filled out, signed and dated, and submitted to City Hall either electronically as a scanned pdf or the original paper form. Photos of a W-9 (in jpeg or other format) are not considered valid submission of a Federal form. You may email a scanned pdf of the form to monica.lake@selahwa.gov or deliver a hard copy to City Hall at 115 West Naches Avenue.
